RULES COMPLIANCE AGREEMENT 

1.        Submit a fully completed a City of Folsom application/roster with all required information by deadline. Only our form will be accepted-please do not create your own or submit an attachment. Applications can be obtained in person at the front desk of the Folsom Sports Complex at 66 Clarksville Rd., and on the City of Folsom website at www.folsom.ca.us, under the Parks and Recreation Department/Folsom Sports Complex.

Resident Team = Minimum 60% residents

2. Roster/Application, a completed Risk Management form for all team coaches, a completed Liability Waiver for each coach and player, a completed Rules Awareness and Compliance Agreement for all coaches, and team fees must be submitted at the time of registration. We accept cash, single check or credit card. Checks should be made out to the City of Folsom. If your team is not accepted, your payment will be refunded. Please note: we will accept payment from only one team representative – we do not accept multiple forms of payment. Receipt of application & fee does not guarantee a spot. Once the registration deadline has passed, if there are still openings, the priority becomes first-come, first-served.

UNLESS YOUR REGISTRATION PACKET IS COMPLETE, YOUR APPLICATION
WILL NOT BE CONSIDERED COMPLETE, AND WILL NOT BE ACCEPTED

We do not register individuals and create teams, only fully formed teams may register. We do provide a free agent list. Please call 984-2850 to add your contact information.

Recreational teams may not have any academy/select/competitive players on the team. Academy, Select or Competitive teams registering to play in the recreational level will be reassigned or removed from league play with a loss of their entire registration fees. Teams who withdraw within 2 weeks prior to league start date will forfeit 50% of their registration fees. Any team that withdraws after the completion of the first week's matches will forfeit all fees. There will be a $25.00 fee for all occasions of teams requesting schedule revisions.

Our leagues are not age pure; they are U8, U10, U12, U14, U16, and U19
