Submit an email to FolsomFire@folsom.ca.us no later than Monday, Oct. 20, with the following information:

• Name, address & telephone number of applicant
• Address where Safe 'n' Sane fireworks will be discharged
• Time of firework discharge
• Number of people expected to attend the celebration
